Community Health Workers: Households and Return Visits
Community health workers can record each household with a UPCS code so follow-up visits, referrals and supervision reach the exact door.

Community health workers walk the same neighbourhoods week after week, often with no street names and no house numbers. They know the way by heart, but the knowledge sits in one person's head. When a worker is sick, transferred or on leave, the next person starts from nothing. A UPCS code writes that local knowledge down as something anyone can open: it takes them to the exact spot that was picked, a 3 m x 3 m square.
The location problem
Home visits are scheduled in advance: antenatal checks, child growth monitoring, chronic-condition follow-up, tracing of missed appointments. Households may be in clusters of near-identical dwellings, up a hill track, or behind a shared yard. Supervisors who want to check work, and referral teams who need to find a patient the worker described, face the same puzzle.
Programmes typically give each worker a catchment area and a register of the households in it. The register is only as useful as its entries are findable: a household number or a family name means little to a replacement worker or an auditing supervisor.
How a UPCS code helps
On the first visit, the worker picks the entrance on the map and records the code against the household ID. From then on:
- The worker's own route is a list of codes.
- A colleague covering the area opens each one for directions.
- A supervisor picking a sample for spot-checks goes to the exact door.
- A referral nurse who has to reach a patient gets a code, not a paragraph.
Putting it into the routine
- Phone browser: paste the code in the search box on upcs.io or open
upcs.io/<code>. - Paper registers: add a column for the code and copy it in during the first visit.
- Programme database: your system can call
POST /v1/upcs/generatefor new households andGET /v1/upcs/lookup/{code}when staff need the position. See the documentation for the request details and thex-api-keyheader. - Training: a two-minute demonstration of zooming in until the grid shows is usually enough.
Practical tips
- Pick the front door or yard gate of the dwelling, not the middle of the compound.
- Use one rule for the whole team so two workers do not choose different points for the same house.
- Keep the code list on a protected device or database, since it sits next to health information.
- Add a landmark as a helpful extra for new staff.
UPCS is a complementary location code and has no health-system integration. For related field routines, read about ambulance and home healthcare and medical lab sample collection.
Frequently asked questions
Is a household's code private?
A code identifies a place, not a person. Treat the list of codes and the names attached to it as sensitive data in your own system and share it only with those who need it.
Can a supervisor find a household from the code?
Yes. Opening the code in the search box leads to the exact spot that was picked, with Get Directions available.
Does this connect to health records?
No. UPCS only provides the location code. Your own forms or records hold the health information.
